Telephone Consultation
We will help you explore and provide information about home health care options. If our services fit your needs or that of a family member, we will schedule an appointment for a home conference.
Initial Home Conference:
In the comfort of your home, one of our professional team members will visit with you, your family and any designated representative to discuss your individual home health care needs.
Registered Nurse Medication Set-up/Treatment Visits:
Visits are scheduled Monday through Friday between 8:30AM-5:00PM to provide medication management and/or treatments.
Home Care Services
Homemaker:
“Hands-off” Services can include light housekeeping, companionship, errand assistance, shopping, transportation to and from appointments, pet care, meal preparation, laundry and much more.
Home Health Aide:
A Home Health Aide can perform all of the skills of the Homemaker plus assistance with bathing, toileting, dressing, grooming, oral hygiene, ambulation, medication assistance and other care as agreed upon.
Home Health Aide Plus:
Additional Services that requires in-home special training for the Home Health Aide such as the use of mechanical lifts.
Home Care Packages
Good Morning Visit:
Visit can include: companionship and peace of mind, assistance out of bed, bathing and dressing assistance, breakfast preparation and medication assistance. Arrival times between 6:30AM-11:00AM. Length of visit is equal to the time needed to complete the care plan.
Good Night Visit:
Visit can include: companionship and peace of mind, dinner clean up, medication assistance, dressing assistance and getting into bed. Arrival times between 6:00PM-9: 30PM. Length of visit is equal to the time needed to complete the care plan.
Bath Visit:
Home Health Aide available Monday through Friday. Arrival times between 9:00AM-3:00PM.
Asleep Overnight:
A Home Health Aide stays up to 10 hours to provide peace of mind to clients and families if something should occur during the night, help is there. Note: This requires that the HHA have separate sleeping arrangements with 8 hours of sleep, 5 hours uninterrupted.
Live-In Home Health Aide:
The Home Health Aide provides light homemaking tasks, personal care, on call services throughout the day. Note: This requires that the HHA have separate sleeping arrangements with 8 hours of continuous sleep, 5 hours uninterrupted.
